	THIS AGREEMENT made and executed between ____________________, herein after referred to as employer, and AUDIO VISIONS, hereinafter referred to as Performers, is mutually agreed by and between both parties as follows:
1.	Performers shall provide Entertainment for Employer at the following time and place. ______________________________________

2.	In consideration for the performance referred to in Paragraph 1, Employer agrees to pay the Performers the amount of $____________, said payment to made at the close of the performance unless otherwise arranged and defined in paragraph 7. Failure to make payment in full as outlined may result in late fees, interest and collection fees.
3.	During the term of this agreement the performers will be responsible to only one person as agent of the Employer. That person shall be:____________________________
4.	This agreement may be cancelled by either party at any time 10 days prior to the performance, provided that the cancellation is received in writing by the Performers or Employer by mail or fax. If the performance is cancelled, within 14 days of the performance, the canceling party agress to pay one-half of the agreement price by the original performance date to the non-canceling party.
5.	Employer agrees to be responsible and pay for all damages caused to Performers equipment by any parties other than Performers and their employees. Employer shall indemnify and hold Performers harmless for any damage or injury that may occur resulting from said performance.
6.	Performers reserve the right to: Control Sound Levels to prevent damage to equipment. Handling of equipment. Deny some requests for music with questionable lyrics or that may not be sutible for the event. (We are happy to adjust the volume down at your request.)
